The contact form asks for your name and email, and if you choose, your phone, what you would like to book, the times that suit you, your insurance company and a message. They are used only to answer you and set up a visit.

Please do not send detailed health information through this form. This policy covers this website only. Ask the clinic how it handles the records from your visits.

Who else sees it
The website is hosted by Cloudflare, a company in the United States, and form messages pass through its email service. Your information is not sold.
How long it is kept
Form messages are kept only as long as needed to answer you and arrange your visit, then deleted.
Your rights
You may ask what is held about you, ask for it to be corrected, or ask for it to be deleted, by writing to hello@example.com. You may also complain to the Office of the Privacy Commissioner of Canada.
